Your application title is to be formatted like this:

LASTNAME, first middle.

Last name is in ALL CAPITAL LETTERS, and the first name in ALL LOWERCASE LETTERS. No, you do NOT capitalize the first letter in the first name. The first name is ALL lowercase letters. It's not a hard concept, but I've had to edit just about every single one that's come through here lately because of that, and I'm getting really tired of it. It even says it in the rules that that's how to format it. AND, if you look at the "accepted" board, they're all like that. Now you have no excuse, because you'll have to have read this to obtain your form. No more excuses.

No, you may not modify the form in any way, or create your own. We did not work our asses off to make this form so that people can just do whatever the hell they want to it.

And for anyone wondering, when it says a number for a minimum, we mean word count (unless it's for a list, such as "likes", in which case we are asking for a certain number of those.)
